Unfractionated heparin (UFH) is a rapid-acting parenteral anticoagulant that potentiates antithrombin III, inhibiting thrombin and factor Xa. It is used for acute thrombosis, STEMI, and bridging therapy. UFH requires frequent monitoring due to unpredictable pharmacokinetics and is the anticoagulant of choice in pregnancy and for patients requiring rapid reversal.
