Low molecular weight heparin (LMWH) with predominant anti-Xa activity used for VTE prophylaxis/treatment and ACS. More predictable pharmacokinetics than UFH with once/twice daily dosing. Not fully reversible with protamine. Contraindicated in HIT and severe renal impairment (CrCl <30).
