Dexmedetomidine is a selective α2-adrenergic agonist providing unique 'cooperative sedation' without respiratory depression, making it ideal for ICU sedation and procedural sedation. The drug reduces sympathetic outflow from the locus coeruleus while maintaining patient arousability. Dexmedetomidine is particularly valuable for neurosurgical patients (maintains CO2 reactivity), alcohol withdrawal, and difficult-to-wean patients. Its analgesic-sparing effects reduce opioid requirements by 30-50%. The drug causes dose-dependent bradycardia and hypotension, especially with loading doses. Dexmedetomidine's short context-sensitive half-time makes it suitable for prolonged infusions without significant accumulation.
